The Nursing Process
The Nursing Process consists of the progression of the nursing behaviors when providing nurse care and implementation.
Assessment
- Involves the gathering of both subjective and objective data about the client’s condition and health status.
Analysis
- Involves interpreting and analyzing the data that was obtained during the assessment phase.
Planning
- Involves making and design ing strategies to correct and or solve problems during the assessment and analysis
Implementation
- Involves initiating and implenting the plan od care. The nurse may perform the care, assist, teach or supervise the client or th client’s family.
Evaluation
- Involves determining the results and the effectiveness of the nursing interventions.
Assessment will always come before implementation. But unless it is a PRIORITY question, then that rule does not apply.

I learned it ADPIE! or Assessment of da pie…Quick Notes: Assess only what you can EAT. Diagnosis by Nurses only-no medical. Plan for the patient. Intervene-what are you going to do for them. Evaluate how they did. Hope this helps.

How to tell the difference in the assessment of subjective vs objective. I always think Subjective of as what the subject or person is telling you. Objective I always think what you observe.
